Modernizing Healthcare Decisions: PalTech’s SMART-on FHIR application system

Jun 26, 2025

Executive Summary

In the fast-paced healthcare sector, timely access to accurate patient data is crucial, not just for informed clinical decisions but also for enabling physicians to treat more patients and drive revenue. A leading Australian healthcare IT consultancy partnered with PalTech to support its expansion into the U.S. market and strengthen its digital health solutions in patient engagement, provider management, and care planning.

Together, we developed a SMART on FHIR-enabled Clinical Decision Support System that delivers real-time insights from EHR data, thereby enhancing provider decision-making, patient engagement, and operational efficiency.

Business Problem

The consultancy faced several critical obstacles that were impeding both clinician efficiency and patient care outcomes:

  • Fragmented Data Access: Patient data was siloed across multiple EHR systems, making it difficult to reliably access complete and up-to-date clinical information.
  • Data Quality Issues: Duplicate records, inconsistent formatting, and outdated data hindered accuracy and clinician trust.
  • Interoperability Gaps: Lack of a unified view and seamless data exchange slowed clinical workflows and decision-making.
  • Workflow Disruption: Integrating new tools without disrupting existing EHR-based workflows was challenging, impacting adoption rates.
  • Population Health Insights: Providers lacked the ability to correlate individual patient profiles with broader demographic trends needed for care planning.
